The procedure may involve the upper eyelids, the lower eyelids, or both. Main indications
Wrinkled and drooping eyelids above the eye are the classic indications for blepharoplasty. The most frequent requests also concern bags under the eyes, dark circles, excess skin or skin slipping on the upper eyelid and eyelashes. These different alterations are responsible for a change in the expression of the gaze.
With upper eyelid plastic surgery, it is possible to correct other defects that alter the look, including: crow’s feet wrinkles (aggravated by this excess skin), lion’s wrinkles or even the sagging of the external part of the eyebrows. For these purposes, injections of hyaluronic acid, fatty tissue by lipofilling or Botox are used.
With lower eyelid plastic surgery, it is also possible to correct the sagging of the cheekbone as well as a swelling called a “malar sac” which forms a double contour under the fatty pockets of the lower eyelid.

The technique
Regarding upper eyelid plastic surgery: excess skin is removed through a hidden incision in the natural crease of the eyelids. The sutures are internal, made with very fine threads placed in the thickness of the skin (intradermal suture) which are therefore invisible. These threads are absorbable and do not require removal.
Regarding lower eyelid plastic surgery: the removal of fat pockets is done through an incision most often placed inside the eyelid (conjunctival route). In addition, the fat pockets can be partly used to fill the depression of the dark circle that descends on the cheek.
If there is significant excess skin on the lower eyelid, it is best to correct this by lateral tightening without scarring under the eyelashes.

Before surgery
An ophthalmological assessment is carried out to look for possible pathologies (insufficient tears, glaucoma, etc.) which may require certain precautions to be taken before or during the procedure, or even contraindicate it.
In the case of general anesthesia, a blood test is carried out and a consultation with the anesthesiologist is mandatory at least 48 hours before the procedure.
Taking medication containing aspirin or anti-inflammatory drugs is prohibited 10 days before the procedure. In case of general anesthesia, it will be necessary to fast for 6 hours before the procedure.
It is recommended to reduce or even stop smoking for one month before and one month after the procedure. Smoking can cause problems with healing.
05.
Blepharoplasty surgery
The procedure lasts approximately 45 minutes for the upper or lower eyelids and therefore 1h30 for all 4 eyelids. It is performed under local, deep local or general anesthesia depending on the case. Hospitalization is limited to half a day in a day hospital.
At the end of the operation, the eyelids are cleaned with physiological serum and left in the open air, without a dressing.

After the intervention
Blepharoplasty is generally a painless procedure. There is often swelling of the eyelid (edema), which varies depending on the case, and is usually accompanied by a small bruise, which disappears after about ten days. However, it is possible to mask them with a covering cream and suitable makeup the day after the operation.
Plan for socio-professional unavailability of 8 to 10 days.
Follow-up visits are carried out within 3 days after the operation, then at 15 days, 1 month, 3 months, 6 months, and 1 year.
Any exposure to the sun is prohibited for one month. It is then possible with covering sunglasses and a sun cream with a maximum protection factor.
In case of lower eyelid surgery via the conjunctival route, wearing contact lenses is not advisable for a week.
The Results
Results of a blepharoplasty
The improvement is immediately visible, the look is refreshed and has retained its identity. The results can already be appreciated after about ten days with stabilization over 1 to 3 months.
In the long term, fat pockets usually do not reappear and sagging skin remains stable for a very long time. The majority of patients do not need to be re-operated. The result is a refreshed, rejuvenated face, giving an undeniable feeling of well-being.
Risks and complications
Apart from the risks inherent in any surgical procedure, complications related to eyelid surgery are very rare: slight hematomas, infection (very rare). Healing anomalies are also very rare because the very thin skin in this area heals well.
Cost of blepharoplasty
The cost of the procedure will depend on the procedure to be performed, the duration of the procedure, the type of anesthesia and any hospitalization costs.
If the reason is purely aesthetic, it does not give rise to reimbursement by Social Security. On the other hand, if it is an act of reconstruction after an accident or illness, it may be covered by Social Security and, where applicable, by your mutual insurance company.
